Please fill out all questions completely.
You must list all players in the trade in one form.
The Additional Information section is for directions for placement of players such as putting them on the
Rookie Squad (3), Injured Reserve (2), or Inactive List (unlimited) when picking them up.

For changing positions of traded players, please use the Free Agent Signing and Positional Change Form.

Each team involved in the trade must fill out and submit the Trading Form for confirmation of the trade.
